The contract that you or your employer negotiated with your insurance carrier defines your dental benefits. We want you to get the most from your dental benefits, and this is best accomplished if you understand them. Please read your benefit or plan booklet or go online to your insurance company website.
Various plans cover endodontic procedures at different levels. As a result your payment portion may vary from our estimate.
We are preferred providers for the listed insurance companies. This means that we have negotiated an UCR (Usual, Customary, and Reasonable) rate with the above companies and will accept that rate. You are responsible for a portion of the payment as defined in your particular contract. We do not know in advance the details of your contract. We, therefore, estimate your portion based on information provided by your insurance company and experience with your insurance company. Any amounts owing by you, or to be refunded to you, will be known after your insurance processes your claim.
If your insurance company is not listed above we can still process your insurance claim. You will be responsible for whatever amount the insurance company does not cover.
